Congratulations to our January Chamber Member of the Month, Kosciusko Home Care and Hospice! Kosciusko Home Care and Hospice is a not-for-profit, independent corporation, Medicare and Medicaid certified, Indiana licensed Home Care and Hospice agency.
Their agency was founded in 1976 by eight local nurses who saw a need for home care services in our community. In 1982, they added hospice services to serve the needs of the terminally ill. Their mission is to provide quality health care promoting the highest level of independence and well being to any who utilize their services. They look forward to continuing to provide Hometown Care for Hometown People!
Kosciusko Home Care & Hospice is located inside the K21 Health Services Pavilion at 1515 Provident Dr, Warsaw.
